Sample Form - Single Page

This is an example of a standard form with the various available field types.

Use the Form Settings to control:
- form title
- form description
- field label placement (global setting, can be override on a per-field basis)
- field description placement (global setting, can be override on a per-field basis)
- sub-label placement (global setting, can be override on a per-field basis)
- custom form classes
- submit button text (and optional conditional logic for display of submit button)
- enabling of the save & continue feature
- limiting the number of entries
- scheduling the form
- restricting the form to logged in users only
- enabling anti-spam honeypot (recommended for all forms)
  • Input masks provide a visual guide allowing users to more easily enter data in a specific format such as dates and phone numbers. More info
  • Section Break

    Forms can be separated into sections using Section Break. Optionally include description / content as well as a section title
  • Please enter a value greater than or equal to 0.
  • HTML Blocks can be added to forms, allowing custom HTML content. See Gravity Form help for more info
  • Advanced Fields

    Advanced Fields include Name, Date, Time, Website, Email, File Upload and List type fields.

  • Post Fields

    Post Fields can be used to allow users to submit data that is then used to populate a Post (e.g. Article)
  • Custom Field types can be set as any of the standard or advanced fields (Single line text, Paragraph text, Drop down, File upload etc)
  • Conditional Logic

    Conditional Logic allows you to configure your form to show or hide fields, sections, pages or even the submit button based on user selections. Entire sections can have their display set conditionally based on one or more other fields in a form. This allows you to easily control what information your user is asked to provide, and tailor the form specifically to their needs.
